Rates

 

If you are paying out-of-pocket (i.e., at a reduced "time-of-service" cash rate because your insurance benefits do not include nutrition visits or you are not using insurance), your appointment fee will be reduced.

If using your medical benefits, insurance companies bill these appointments at $250.00+ and $210.00+, respectively.

Note: Payment is due at the appointment time for each visit type; cash, check, debit, and credit cards accepted. For debit and credit card transactions, a 3.5% card-convenience processing fee is assessed.
